Instructions

In order to download and complete an automated form, please follow these steps:

  1. RIGHT CLICK on the link you wish to download and select SAVE TARGET AS . This function will allow you to save the file to your PC.
  2. Select the appropriate location on your PC where you wish to save the file and Click SAVE.
  3. Once the file is downloaded, go to that same location on your PC and open the Excel file for use.
  4. You may be prompted with a message box asking if you want to enable to disable the macros; select the option to ENABLE THE MACROS .
  5. These report forms contain several security and validation features. Users are only allowed to enter information in the appropriate data cells. While completing the form, you may encounter a series of validation pop-up messages that will not allow the user to enter invalid data. In the event the user does encounter an error(s) while completing the form, we recommend the user to select the "Validation Rules and Edits" page to view the validation matrix. The matrix will provide information to indicate where the invalid data can be located.
  6. Once the document is completed, you can simply click on the EMAIL button located at the top of the Report Form. This will open your default software program and automatically attach the completed Excel file, insert report information in the subject line, and insert an assigned email address for that specific report. Simply click SEND to transmit the email and the process is complete.

However in the event the report form contains invalid data, the EMAIL button will place a "RED CIRCLE" around all the invalid data. You will be required to re-enter valid data before the program will allow you to email the document. We recommend the users select the "Validation Rules and Edits" sheet tab to view the validation matrix. The matrix will provide information to indicate where the invalid data can be located.
