National Disqualified List

The Child and Adult Care Food Program regulations require that any program operator, responsible principal/individual (RP/I), or Day Care Home (DCH) provider that has been terminated and disqualified in the serious deficiency process be placed on the U.S. Department of Agriculture’s (USDA) National Disqualified List (NDL).

The CDSS will forward all terminations and disqualifications to the USDA Food and Nutrition Service (FNS) regional office for review. The following information is required when submitting information to FNS for inclusion on the NDL:

For Program Operators and Responsible Principals/Individuals

  • Name and address of the program operator including city, state, and zip code
  • Any known program operator alias(es)
  • Termination date
  • Amount of debt owed, if any, noting the RP/I as responsible for the debt
  • Reason(s) for the disqualification
  • RP/I’s full name
  • Any known RP/I alias(es)
  • RP/I’s address including city, State, and zip code
  • RP/I’s date of birth
  • RP/I’s position title in the program

For DCH Providers

  • Name of the provider
  • Any known alias(es)
  • Address including city, State, and zip code
  • Date of birth
  • Termination date
  • Amount of debt owed, if any
  • Name and address of the provider’s DCH Sponsor
  • Reason(s) for the disqualification

If the CDSS has identified a program operator as seriously deficient, or if a DCH Sponsor has identified a provider as seriously deficient, and the program operator or provider self-cancels before correcting the identified serious deficiencies, the program operator or provider is still responsible for correcting the serious deficiencies and will be terminated and disqualified if they do not complete the corrective action to fully and permanently correct the serious deficiencies.
