Managed Care Plan Enhanced
Care Management and Community Supports Referral Pathways
Managed Care Plans (MCPs) are responsible for administering both the Enhanced Care Management (ECM) and Community Supports (CS) initiatives in collaboration with their network of community-based providers.

ECM and CS are separate initiatives, and some Medi-Cal Members may qualify for only ECM, or only CS. To be eligible for ECM, Members must be enrolled in a Medi-Cal MCP and meet at least one of the ECM Populations of Focus. Children involved in (or with a history of involvement in) child welfare, including foster care up to age 26 are a population of focus. CS are available to eligible Medi-Cal members regardless of whether they qualify for ECM services.

For Community Supports, Members should call HPSM Member Services at 650-616-2133.
If they are not eligible, HPSM can help them find other supports.

CS, with the exception of Transitional Rent, which is mandatory for MCPs to offer beginning January 1, 2026, are optional for MCPs to offer and for Members to utilize. Meaning not all CS are offered at each MCP.
DHCS encourages MCPs to offer as many of the 14 pre-approved CS as possible. To find the Community Support options for your Medi-Cal Plan you can enter your county into a search bar to learn what community supports your Medi-Cal plan offers here.
