License Number
The License Number field indicates the license number of the provider. For license-exempt providers, this
information field should be left blank.
Where to Find It
The Provider License Number can be found on the license from the California Department of Social Services, Community
Care Licensing.

Only leave this field blank if the provider is license-exempt AND the Type of Care reported in the CDD-801A is
one of the following:
- 05 - License-exempt in child’s home – relative
- 06 - License-exempt in child’s home – non-relative
- 07 - License-exempt outside child’s home – relative
- 08 - License-exempt outside child’s home – non-relative
- 11 - License-exempt center-based care

The provider License Number must contain nine (9) digits.
- In Range: Starts with 01 – 58
- Out of Range: Starts with 00 or 59 and above
- Leading zeroes must be included.
- Enter the license number as it appears on the license from the CDSS, Community Care Licensing.

Frequently Asked Questions
- I’m a license-exempt provider, so I left the license field blank, however, when I try and save the CDD-801A
family file, I receive an error message.
- Verify the Type of Care reported is license-exempt.
- What license should I use if a child’s age changes mid-month, so would move licenses:
- The age of the child on the last day of the month is the license they should
be reported under.
- What license should I use when the ages overlap for licenses?
